What's Irinotecan?

Irinotecan is a semi-synthetic water-soluble camptothecin derivative. This product and its metabolite SN38 are inhibitors of DNA topoisomerase I. The complex formed with topoisomerase I and DNA can cause DNA single-strand breaks, prevent DNA replication and inhibit RNA synthesis, which is the S phase of the cell cycle. Specificity.

Function of Matrine

Large-scale clinical trials have shown that irinotecan hydrochloride has significant inhibitory effects on a variety of tumors such as colon cancer, small cell lung cancer, rectal cancer, leukemia, etc., and is still effective against fluorouracil-resistant cases. Therefore, irinotecan hydrochloride has attracted widespread attention around the world.

Application

It is a first-line drug for advanced colorectal cancer and can also be used for postoperative adjuvant chemotherapy; it also has certain effects on lung cancer, breast cancer, pancreatic cancer, etc.

Adverse reactions

1. Delayed diarrhea

The incidence rate is 80% to 90%, of which 39% are severe cases. The median time of onset is the 5th day after taking the drug, with an average duration of 4 days. Severe cases can be fatal. Diarrhea that occurs 24 hours after taking the drug should be regarded as delayed diarrhea. Once the first watery stool or abnormal abdominal motility occurs, oral loperamide should be started immediately. The first dose is 4 mg, and then 2 mg, once every 2 hours. At least 12 hours, and continue to take the medicine for 12 hours after the last watery stool, and the maximum time of taking the medicine should not exceed 48 hours. If diarrhea persists after 48 hours, prophylactic oral broad-spectrum antibiotics (quinolones) should be started for 7 days and other antidiarrheal treatments should be used.

2. Severe neutropenia

The incidence rate is 39.6%. Gastrointestinal reactions (nausea, vomiting) are common but not serious. Prophylactic use of filgrastim (G-CSF) or sargramostim (GM-CSF) is not recommended for neutropenia. If severe delayed neutropenia occurs in the first week, its use may be considered.

3. Other acetylcholine syndrome

Includes "early" diarrhea (appearing within 24 hours), sweating, increased salivation, visual disturbances, spastic diarrhea, and tearing. If the above symptoms are severe, atropine 0.25 mg can be given subcutaneously. Prophylactic atropine is not recommended in the first cycle, but if severe symptoms, including early diarrhea, occur, atropine 0.25 mg subcutaneous injection can be given prophylactically in the next cycle. Mucositis, alopecia, fatigue, and skin toxicity were all mild.

4. Effect on liver function

In patients without progressive liver metastases treated with monotherapy, the incidence of transient and mild to moderate elevations in serum aminotransferase, alkaline phosphatase, and bilirubin levels was 9.2%, 8.1%, and 1.8%, respectively. Transient mild to moderate elevations in serum creatinine levels occurred in 7.3% of patients; transient (grade 1 and 2) serum AST, ALT, alkaline phosphatase, or The incidence of elevated bilirubin levels was 15%, 11%, 11% and 10% respectively. The incidence of transient grade 3 elevations was 0%, 0%, 0%, and 1% of patients, respectively. No 4 degree rise was observed.
