Function

Clomipramine produces antidepressant effects by inhibiting the reuptake of norepinephrine (NA) and 5-hydroxytryptamine (5-HT) in the presynaptic membrane, as well as anxiolytic and sedative effects. The inhibitory effect of 5-hydroxytryptamine reuptake is stronger than that of other tricyclic drugs, while the inhibitory effect of norepinephrine reuptake is less than that of imipramine and amitriptyline. Its anticholinergic effect is moderate and its sedative effect is low. It is well absorbed after oral administration. The therapeutic blood concentration is 200-500ng/ml, and the steady-state blood concentration can be reached in 1-2 weeks. After hepatic metabolism, the concentration of the active metabolite norclomipramine is twice the concentration of the original drug. The plasma protein binding rate is 96Chemicalbook~97%, and the half-life is 21~31 hours. About 70% is excreted in urine and 30% is excreted in feces. It is suitable for the treatment of endogenous, reactive, neurological, latent depression and various depressive states; schizophrenia accompanied by depression; obsessive-compulsive disorder, phobia; various pain, etc. The drug can also reduce the levels of 5-hydroxyindoleacetic acid (5HIAA) in cerebrospinal fluid and platelets, which is related to the drug's ability to treat obsessive-compulsive disorder. It has affinity for D2 receptors, alpha-adrenergic receptors, and histamine receptors. In addition, it has strong anticholinergic effects and mild dopamine antagonism that can increase plasma levels of prolactin and growth hormone.

Drug interactions

Concomitant use with ethanol can cause central nervous system depression.

Concomitant use with anticonvulsants can lower the epilepsy threshold, thereby reducing the effect of anticonvulsants, and the dosage of antiepileptic drugs must be adjusted.

When used together with antihistamines or anticholinergics, the drug effects will strengthen each other, and the dosage needs to be adjusted in time.

When guanethidine and clomipramine are used together, the antihypertensive effect of the former may be reduced. However, when the dosage does not exceed 150mg per day, the efficacy of guanethidine will not be affected by Chemicalbook.

When used together with estrogen or estrogen-containing contraceptives, it can increase the adverse reactions of clomipramine and reduce the antidepressant efficacy.

Combined use with monoamine oxidase inhibitors can cause hypertensive crisis, and death has been reported. Generally, the former should be stopped for two weeks before use.

Combined use with adrenergic receptor agonists may cause severe hypertension and high fever.

Combined use with thyroid preparations can synergize with each other and cause arrhythmia, so the dosage of both must be reduced.
