Roflumilast, a phosphodiesterase-4 (PDE-4) inhibitor, is a new treatment drug for COPD. It is used to treat symptoms of bronchitis-related cough and mucus hyperplasia in patients with severe COPD.

Function

Roflumilast is orally administered and metabolized into N-oxide by cytochrome oxidase P450(CYP)3A4 and CYP1A2 enzymes after entering the body. The activity of roflast N-oxide is only 2-3 times weaker than that of roflast, but also has higher PDE-4 selectivity. About 90% of the PDE-4 inhibitory effect in human is produced by roflast N-oxide, and the other 10% is produced by roflast prototype. The free plasma concentration of roflurast n-oxide was about 1 ~ 2nmoChemicalbookl· l-1 after 24h of oral administration of roflurast 500μg once daily in healthy subjects, and the plasma protein binding capacity of roflurast n-oxide was about 97%. Smoking had little effect on the pharmacokinetics of rofluast. Roflast is mainly expressed in inflammatory cells associated with asthma, including eosinophils, neutrophils, and mast cells. This drug can specifically act on certain enzymes involved in smooth muscle contraction, prevent cAMP degradation, thereby blocking proinflammatory signal transmission, and has anti-inflammatory activity. It has obtained good curative effect in the clinical treatment of asthma and chronic obstructive pulmonary disease. Roflast also significantly delays the deterioration of respiratory symptoms and greatly improves the quality of life of patients.

It is used for the prevention and treatment of respiratory diseases of livestock and poultry, and the treatment of severe chronic obstructive pulmonary disease (COPD) and bronchitis with excessive mucus cough, asthmatic bronchitis, bronchial asthma, acute and chronic bronchitis with abnormal secretion of respiratory tract, airbag inflammation, etc.
