Paroxetine, commonly used as hydrochloride or mesylate. Hydrochloride is white or off-white, easily deliquescent crystalline powder, soluble in methanol, slightly soluble in anhydrous ethanol and methylene chloride, slightly soluble in water. Methanesulfonate is a white powder, odorless, solubility in water >lg/ml, melting point 147 to 150 °C. It is a selective serotonin reuptake inhibitor (SSRI) type antidepressant, its drug form is Paroxetine hydrochloride, Chinese trade name "Seroxet" or "Ke Worry" (Seroxat). It is commonly used in the treatment of social phobia, depression and obsessive-compulsive disorder in clinical practice.

Paroxetine is an antidepressant that belongs to the category of selective serotonin reuptake inhibitors (SSRIs). Paroxetine affects chemicals in the brain that can be out of balance in people with depression, anxiety, or other conditions.

Paroxetine is mainly used to treat depression, including major depressive disorder. It is also used to treat panic disorder, obsessive-compulsive disorder (OCD), anxiety disorder, post-traumatic stress disorder (PTSD), and premenstrual dysphoric disorder (PMDD). Paroxetine, (1) is used for the treatment of depression. It is suitable for the treatment of depressed patients with anxiety disorders, the effect is faster than TCAs, and the long-term effect is better than that of imipramine. (2) It can also be used for the treatment of protophobia, social phobia and obsessive-compulsive disorder.

 

Product Application 

This product is a phenypiperidine derivative, which is an SSRI, which can selectively inhibit the 5-HT transporter, block the reuptake of 5-HT by the presynaptic membrane, prolong and increase the effect of 5-HT, thereby producing antidepressant effects. At the usual dose, there was no significant effect on other transmitters except for a weak inhibition of the reuptake of NA and DA. It can be completely absorbed after oral administration, with a bioavailability of 50%, and its absorption is not affected by food or drugs. There is a first-pass effect. The plasma half-life is 24 hours, and the half-life is extended in the elderly. The plasma protein binding rate is 95%. It can be distributed in all tissues and organs of the body, and can also be secreted through the breast. It is mainly metabolized by the liver to produce uridine compounds, some of which are mediated by CYP2D6. Finally, it is excreted from the body through the kidneys, and a small part is excreted from the feces through bile secretion. Paroxetine, (1) is used for the treatment of depression. It is suitable for the treatment of depressed patients with anxiety disorders, the effect is faster than TCAs, and the long-term effect is better than that of imipramine. (2) It can also be used for the treatment of protophobia, social phobia and obsessive-compulsive disorder.

 

Precautions
(1) MA0Is cannot be used within two weeks before and after taking this drug.

(2) Patients with a history of epilepsy or mania, angle-closure glaucoma, bleeding tendency, suicidal tendencies or a history of severe depressive state should be used with caution. It can still be used safely in patients with hepatic and renal insufficiency, but the dose should be reduced.

(3) After one-time administration, there may be a slight slowing of heart rate and blood pressure fluctuations, which are generally not clinically significant, but the reaction should be paid attention to for patients with cardiovascular disease or newly discovered myocardial infarction.

(4) It can only be effective after 1 to 3 weeks of use. It is long enough to consolidate the effect. Depression, obsessive-compulsive disorder, and panic disorder are all treated with longer maintenance periods.

(5) It has been reported that rapid discontinuation of the drug can cause syndrome: sleep disturbance, irritability or anxiety, nausea, sweating, confusion. In order to avoid discontinuation reactions, a withdrawal regimen is recommended: according to the patient's tolerance, if tolerated, the dose should be reduced at a rate of 10 mg per week, and the dose to 20 mg per day should be maintained orally for 1 week before discontinuation. If it cannot be tolerated, the reduced dose can be reduced, and if the patient responds strongly, a reversion to the original dose may be considered.
