Isoniazid, also known as 4-pyridinecarbohydrazide and isonicotinic acid hydrazide, is an organic compound with the chemical formula C6H7N3O, a hydrazide of isonicotinic acid, a white crystalline powder, and a first-line anti-tuberculosis drug, along with rifampicin, ethambutol, and pyrazinamide.4-Pyridinecarbohydrazide is a derivative of pyridine that contains an amide functional group. The amide functional group is a very important functional group in organic compounds. 1.Anti-tuberculosis
It has inhibitory and killing effects on Mycobacterium tuberculosis, with good biofilm penetration, and is classified as the first choice anti-tuberculosis drug due to its good efficacy, low toxicity, inexpensive price, and convenient oral administration. The oral absorption rate of isoniazid is 90%; the serum concentration of the drug can reach the peak in 1~2 hours after serving; the Vd is 0.61±0.11L/kg, and the protein binding rate is very low. The product is metabolized in the body mainly by acetylation with partial hydrolysis. Due to genetic differences, the population can be divided into fast acetylators and slow acetylators. Their half-lives are significantly different, with an average t1/2 of 1.1 hours for fast acetylators. For slow acetylators it is 3 hours. The product readily crosses the blood-brain barrier. It is mainly used for all types of tuberculosis in the progressive stage, dissolution and dissemination stage, absorption and improvement stage, and can also be used for tuberculous meningitis and other extrapulmonary tuberculosis, etc. It often needs to be combined with other anti-tuberculosis drugs. The product often needs to be combined with other anti-tuberculosis drugs to enhance the efficacy and overcome drug-resistant bacteria. In addition, it is also effective in dysentery, whooping cough, and erysipelas.
2.Antidepressant
Isoniazid was also the first antidepressant, but was withdrawn from the market because of strong liver toxicity.
3.Antibacterial
Isoniazid is highly selective against Mycobacterium tuberculosis and has strong antibacterial effect. It can inhibit bacteria at a concentration of 0.025~0.05mg/L in test tube, and the higher concentration of 10mg/L can have bactericidal effect against the bacteria in the reproduction stage. The higher concentration of 10mg/L has bactericidal effect on the bacteria in the reproduction stage. It has the same killing effect on intracellular and extracellular Mycobacterium tuberculosis. Isoniazid alone is susceptible to drug resistance, but the combination of drugs can delay the development of drug resistance and enhance the efficacy of treatment. Isoniazid has no cross-resistance with other antituberculosis drugs.
The mechanism of action of isoniazid is not well understood, although it has been studied in detail. One theory is that isoniazid is converted to isonicotinic acid, which acts as an antimetabolite of nicotinic acid and binds to NAD+ instead of nicotinic acid, and the spoofed NAD+ fails to catalyze normal redox reactions. Another theory is that by blocking the action of desaturase, isoniazid inhibits the conversion of C24 and C26 saturated fatty acids to C24 and C26 unsaturated fatty acids, and these unsaturated fatty acids are most likely the precursors of mycolic acid, which is a key component of the bacterial cell wall, and inhibition of mycolic acid biosynthesis results in the loss of bacterial acid resistance, a mechanism which amply demonstrates that isoniazid's effect on Mycobacterium tuberculosis This mechanism fully demonstrates the effect of isoniazid on the cell wall of Mycobacterium tuberculosis.
