Product Introduction

Ivabradine hydrochloride is the first selective and specific If channel blocker developed by the French company Servier with a simple heart rate-lowering effect for the symptomatic treatment of chronic stable angina with normal sinus rhythm and contraindication or intolerance to β receptor blockers. Unlike B-receptor blockers, which are the most commonly used angina treatments, they do not cause airway contraction or spasm.

Product Function

Ivabradine hydrochloride is a hydrochloride synthesized from Ibarradine with one mole of hydrochloric acid and is used to treat patients with angina who cannot tolerate - receptor blockers and/or heart failure.

 

Dosage

The usual recommended starting dose is 5 mg twice a day, morning and evening meals. If the patient's resting heart rate is persistently above 60 beats/minute, the dose should be increased to 7.5 mg twice daily if the patient's resting heart rate is persistently below 50 beats/minute or if the patient has symptoms related to bradycardia, and if the patient's heart rate is between 50 and 60 beats/minute, the dose should be maintained at 5 mg twice daily. During treatment, if the patient's resting heart rate is persistently below 50 beats per minute, or if symptoms related to bradycardia develop, the dose of 7.5 mg or 5 mg twice daily should be lowered to the next lower dose. If the patient's resting heart rate is persistently higher than 60 beats per minute, the dose of 2.5 mg or 5 mg twice daily should be increased to the last higher dose. Patients with renal insufficiency and creatinine clearance greater than 15ml/min do not need to adjust the dose, and there is no clinical data on the use of this product in patients with creatinine clearance less than 15ml/min, and such people need to be cautious when taking drugs. Patients with mild liver impairment do not need to adjust the dose, patients with moderate liver impairment need to be cautious when using this product, and patients with severe liver dysfunction are prohibited from using this product.

 

Side effects

The most common adverse reactions are flash of light (photovisual hallucinations) and bradycardia, which are dose-dependent and related to the pharmacological effects of ivabradine. The more common adverse reactions also include: neurological diseases, such as dizziness and headache; blurred vision; Heart disease, such as bradycardia, first-degree atrioventricular block, extraventricular contractions, atrial fibrillation; Poor blood pressure control.
Uncommon or rare adverse reactions include: eosinophilia, hyperuricemia, syncope, diplopia, visual disturbances, palpitations, supraventricular extraperiodic contractions, hypotension, dyspnea, nausea, vomiting, diarrhea, angioedema, rash, muscle spasms, and elevated serum creatinine.

 

Special populations

Women of childbearing age: Women of childbearing age should use appropriate contraception during treatment. Pregnant women: There are limited data on the use of this product in pregnant women, but animal studies have shown that ivabradine has reproductive toxicity, embryotoxicity and teratogenic effects, therefore, pregnant women are contraindicated.
Lactating women: animal studies have shown that ivabradine can be secreted into breast milk and is therefore contraindicated in lactating women.
Children: There are no data on the use of this product in children under 18 years of age. Elderly: Older patients aged 75 years or older should consider starting at a lower starting dose (2.5 mg) and adjusting the dose if necessary.
