Product Introduction

Analgin (sodium salt, Chinese generic name, translated from the Slavic national trade name), with Western generic name metamizole (INN) [a] or dipyrone, Japanese and Korean generic name sulpyrine, is an analgesic, antispasmodic, and antipyretic. Analgin is most commonly administered orally or by intravenous infusion. It belongs to the 4-aminoantipyrine sulfonate family of drugs.

Sales of Analgin are restricted in some jurisdictions after studies in the 1970s linked it to serious adverse effects, including agranulocytosis. Newer studies dispute this judgment, claiming it is safer than other analgesics. A 2021 Brazilian retrospective review concluded that Analgin was banned because of the contamination of its structurally similar aminopyrine, and that there was no evidence at the time that Analgin was particularly harmful. 

 

Although Analgin is a relatively safe drug, it is not completely free of adverse effects.

Analgin has potential for blood-related toxicity (blood lesions), but it is less toxic to the kidneys, circulatory system, and gastrointestinal tract than NSAIDs. Like NSAIDs, it can trigger bronchospasm or severe allergic reactions, especially in those with asthma.

Serious side effects include agranulocytosis, aplastic anemia, hypersensitivity reactions (such as severe allergic reactions and bronchospasm), toxic epidermal necrolysis, and it may also induce acute attacks of porphyria because it is chemically related to sulfonamides. The relative risk of agranulocytosis appears to vary greatly depending on the country in which the incidence is estimated, and there is a strong disagreement about the risk. Genetic factors may play an important role in Analgin sensitivity. Some people think that certain populations are more susceptible to Analgin-induced agranulocytosis than others. For example, analgin-related agranulocytosis seems to occur more frequently in the British population than in the Spanish population. An assessment by the European Medicines Agency noted that "the potential to induce agranulocytosis may be related to the genetic characteristics of the population studied".

A 2015 meta-analysis concluded that based on the available evidence, "for short-term use in a hospital setting, Analgin appears to be a safe option compared with other widely used analgesics", but that "the results were limited by the overall mediocre quality of the reports analysed".

A 2016 systematic review found that Analgin significantly increased the relative risk of upper gastrointestinal bleeding by 1.4–2.7 times. Analgin's effect on colic has been attributed to its analgesic properties, with no evidence that it interferes with small bowel or colonic motility.

A study conducted by one of the drug's manufacturers found the risk of agranulocytosis within the first week of treatment to be 1.1 per million, compared with 5.9 per million for diclofenac.
